Clinical Management in Physician Practice During the Transformation of the Healthcare System in Ukraine: Conceptual and Methodological Foundations

Abstract

Purpose. The aim of the study is to systematize the conceptual and methodological foundations of clinical management as an integrative mechanism for aligning clinical and managerial approaches in physicians’ professional activities under the transformation of the healthcare system in Ukraine in accordance with European Union standards. Materials and methods. The study has a theoretical and methodological design and was conducted using historical-retrospective, system-structural, bibliographic, comparative, and content analysis methods. These approaches enabled the conceptualization and systematization of the foundations of clinical management, as well as the generalization of its developmental patterns in both global and national contexts. Results. Within the scope of the study, the conceptual and terminological foundations of the category «clinical management» were refined. It is defined as an integrative concept combining physicians’ clinical reasoning with managerial instruments aimed at ensuring the quality, safety, and effectiveness of healthcare delivery. The etymological basis of the terms “clinical” and “management” was substantiated, reflecting the integration of individualized clinical decision-making with systemic management of healthcare resources and processes. It was established that clinical management represents an integrated system of managing clinical processes at the micro level, ensuring alignment of clinical decisions with modern managerial approaches and being grounded in the principles of evidencebased medicine. Its conceptual differentiation from related categories – clinical governance, healthcare management, and medical management – was defined, as these operate at institutional, strategic, and administrative levels, respectively. A retrospective analysis demonstrated the evolution of clinical management from clinical audit and treatment outcome evaluation to contemporary process-oriented and digitally integrated models of healthcare quality management. Key drivers of this evolution include quality management theories, evidence-based medicine, and digital health technologies. It was shown that the implementation of clinical management in Ukraine is determined by a complex set of historical, socioeconomic, and institutional factors, including healthcare system transformation, resource constraints, increasing burden of chronic diseases, population ageing, and the need for standardization of clinical decision-making, as well as the transition to European standards of quality, safety, and healthcare organization. The institutionalization of international approaches to healthcare quality and patient safety has also played a significant role. A staged transformation of clinical management in Ukraine was identified, ranging from the introduction of clinical protocols to digitalization of processes and development of analytical systems, enabling a patient-centered model of healthcare delivery. The findings confirm that clinical management is a key integrative component of modern medical practice and provides a theoretical foundation for further development of an organizational and managerial model for its implementation. Thus, the conducted analysis identified the evolutionary prerequisites for the formation of clinical management as an integrated system of clinical process governance and provided a theoretical basis for developing the author’s organizational and managerial model, its implementation, and performance evaluation, which are presented in the second part of the study. Conclusions. It has been established that clinical management is an integrated system for managing clinical processes aimed at improving the quality, safety, and efficiency of healthcare delivery. It has been proven that its development evolves from administrative coordination toward systematic, standardized, digital, and patient-centered management. It has been argued that in Ukraine its formation is driven by healthcare system reforms, digital transformation, and the implementation of European standards. The obtained results form the theoretical basis for developing an organizational and managerial model of clinical management.
